DR Congo rolls out Ebola vaccine for health workers as death toll rises

The Bundibugyo virus outbreak in the Democratic Republic of the Congo (DRC) has expanded to one additional health zone, Kayna, in North Kivu, increasing the total number of affected health zones to 61 across six provinces: Bas-Uélé, Haut-Uélé, Ituri, North Kivu, South Kivu, and Tshopo. Additional confirmed cases and deaths were reported only in the DRC, with a cumulative total of 6,778 confirmed cases: 6,757 in the DRC (including two cases diagnosed in the DRC and treated in Germany), 20 in Uganda, and one in France. A total of 3,269 deaths were reported, including two in Uganda. As of 7 September, at least 1,611 patients had recovered, including 1,590 in the DRC, 18 in Uganda, two in Germany, and one in France. The sustained transmission in the DRC poses a risk of cross-border spread, with health screening and surveillance activities ongoing.

The outbreak, concentrated in Ituri province, is the deadliest of 17 past Ebola outbreaks in the DRC. The vaccination campaign in Ituri and North Kivu is expected to cover 20,000 frontline workers over six to nine months. The rollout initially targeted Bunia and Mongbwalu zones in Ituri, which are among the hardest-hit areas. Last month, the Ervebo vaccine was distributed in Kisangani, Tshopo province, with plans to cover 14 health zones in Tshopo, Bas-Uélé, and Haut-Uélé. WHO experts cited early data from animal trials suggesting the vaccine could offer some protection in the current outbreak.

The DRC’s health ministry and international partners have emphasized the importance of timely case detection, access to care, and transmission interruption to curb the outbreak. Delayed detection continues to increase the risk of spread within households, communities, and healthcare settings. The high CFR underscores the challenges in managing the crisis, particularly in areas with limited healthcare infrastructure. Cross-border collaboration remains critical, as the outbreak’s geographic expansion and sustained transmission threaten regional stability. Health screening and surveillance activities are being intensified to mitigate the risk of further spread.

The virus has also affected neighboring countries, with 20 cases in Uganda and one in France. The Ervebo vaccine’s deployment highlights the urgency of protecting frontline workers, who face heightened exposure due to their roles in containment efforts. The vaccine’s use against the Bundibugyo strain remains unproven, but its administration reflects the lack of alternative options. Ongoing clinical trials aim to determine its efficacy, with results expected to inform future vaccination strategies. The DRC’s response continues to rely on international support, including funding, technical expertise, and logistical assistance from WHO, MSF, and other organizations.

The outbreak’s impact extends beyond public health, straining local economies and displacing communities. Health workers report increased pressure to manage rising caseloads while navigating safety concerns. Community engagement remains a priority, as misinformation and resistance to interventions can hinder containment efforts. Local leaders and health authorities are working to build trust through education campaigns and transparent communication. The situation underscores the need for sustained investment in healthcare systems to prevent future outbreaks and improve emergency response capabilities.

The DRC’s government, in collaboration with international partners, has committed to scaling up vaccination efforts and expanding surveillance. The Ervebo vaccine’s role in the current outbreak remains under evaluation, with its effectiveness against the Bundibugyo strain requiring further study. As the outbreak evolves, the focus remains on reducing transmission, supporting affected communities, and strengthening global preparedness for emerging health threats. The high CFR and persistent challenges highlight the urgency of coordinated action to prevent further loss of life and mitigate the outbreak’s long-term consequences.
